Our team provides one-on-one support tailored to each individuals needs while they are at work. This includes personal care assistance, behavioral support, and on the job follow alongs.

Our team provides eligible members work in real community job settings as part of a small, supervised group. We're job coaches ready to help!

Our team provides eligible members live in support services in their own homes. This includes personalized, person centered support to help each individual increase or maintain their independence in the day to day.

Our team helps eligible members who are unable to get to and from DDD approved programs, appointments, day services, or their place of work in a non-emergency, on demand rides, all on their own!

This service is a time limited DDD service that our team provides. Designed to help eligible members build the foundational skills and readiness needed to work in the community.
Consultation
Counseling
At Five Pillars Care we know that home health care is more than physical support, it's about emotional well-being too. That's why we offer counseling so that clients and their families navigate challenges, make informed care decisions, and feel supported every step of the way. Have a concern? We will give it a listen!
Faith Based Counseling
Outside of the emotional support that we readily offer, we also are aware that everyone needs their spiritual well-being cared about as well. We provide guidance, comfort, and support aligned with our client's personal beliefs. We are all human at the end of the day. Compassion is the word!
Group Therapy
At Five Pillars Care we understand that connection and shared experiences are a vital part of healing and growth. We're here to provide a safe, supportive space where clients can share challenges, learn from others, and build coping skills together. We are here to foster a broad, tight knit community.
Trainings and Certifications
At Five Pillars Care, we believe that high quality care starts with well prepared caregivers. That's why we provide trainings and certifications for first aid/cpr, prevention and support, and article 9. We want to ensure our team is equipped with the necessary knowledge, skills, and confidence to provide care safely and effectively.
